简介

尽管推荐进行结肠直肠癌筛检的做法已经很普遍，但在美国，筛检率仍然较低。本研究的目的是通过研究两个类型的小组，即1）曾经进行了筛检的人士和2）按时进行筛检的人士，来描述使用结肠直肠癌筛检的比率和预测指标，并评估发展趋势和预测指标是否随时间变化。

方法


我们分析了2000年和2003年全国健康访问调查（National Health Interview Survey）中，50岁及以上没有结肠直肠癌病史的成年人使用大便隐血测试、乙状结肠镜和结肠镜的数据（2000年N = 11,574，2003年N = 11,779）。

结果

2000年研究群体中曾经进行了结肠直肠癌筛检人的比例（53%），在2003年的研究群体有所提高（55%），而2000年研究群体中按时进行结肠直肠癌筛检人的比例（38%）在2003年也同样有所增加（53%）。在曾经进行了筛检的人中，76%于2003年按时进行了筛检，而2000年只有68%。结肠镜的使用有所增加，但大便隐血测试和乙状结肠镜的使用则有下降。收入较高、教育程度较高、有保险、有惯常照护来源而且前一年曾看过牙医的人与不具有这些预测指标的人相比，更有可能按时进行筛检。自2000年以来，这些结肠直肠癌筛检使用的预测指标一直保持稳定。

结论


尽管在提高总体筛检率方面取得的成功相对有限，但值得鼓励的是曾经进行过筛检小组中的大多数人都按时进行了结肠直肠癌筛检。尽管筛检政策和比率有所变化，然而结肠直肠癌筛检的预测指标随时间推移保持稳定。需要开展进一步的研究以揭示结肠直肠癌筛检的阻碍因素。
