Errata: Vol. 46, No. RR-3

The MMWR Recommendations and Reports "Poliomyelitis Prevention in the United States: Introduction of a Sequential Vaccination Schedule of Inactivated Poliovirus Vaccine Followed by Oral Poliovirus Vaccine -- Recommendations of the Advisory Committee on Immunization Practices (ACIP)," contained two errors. On page 14, two footnote markers on Table 4 were incorrect; below is the corrected version of (Table_4). On page 25, reference number 62 should read: "Chen RT, Rastogi SC, Mullen JR, et al. The Vaccine Adverse Event Reporting System (VAERS). Vaccine 1994;12:542-50."
